Case Management is formally described as ‘a process devoted to the coordination, rehabilitation, care and support of people with complex, clinical needs. It aims to facilitate their independence and improve their quality of life; whilst acknowledging safety issues’. In practical terms and from Don’s experience this means the Case Manager:
- Acts as the Client Advocate
- Acts as Team ‘Conductor’
- Recruit Therapists
- Recruit Rehab Support Workers
- Facilitate Worker Training
- Facilitation of Team Goal Planning
- Plans/Monitors Service Delivery
- Is Implementer of ‘Expert’ Recommendations
- Researches Services/Facilities/Opportunities
- Liaises with Key Others
- Family
- Solicitor
- Receiver
- Statutory Services
- Is ‘Trouble Shooter’!!!
Working with and supervised by Don in carrying their own case load is a number of Employed and Associate Case Managers whose collective skills and knowledge offer clients an exceptional level of expertise. The Associates based in locations across the south east enable BIS to meet the needs of individuals who would otherwise be too distant to benefit from our service. The Team network also ensures that continuity of service is provided out of hours and during the absence of the primary case manager.
